I need an internet portal which is based on the same technology like http://equestionnaire.de
The functionality is a bit different: manage questions, get questionare to online users, show results based on answers. The new questions are dependent on the answers to previous questions. The questions should be editable and insertable online. There is some business logik, which is corresponding to the questions.

I don't have the specification. What I have is just an example in Internet which should be the same like what I need. I have also a java-swing application (prototype), which could be used to get some information and to understand what I need.

It should be PHP web-application.
The project is remote job.
